The Church has always understood the “brothers and sisters” of Jesus as not referring to other children of the Virgin Mary. CCC 500

James and Joseph, called “brothers of Jesus,” are the sons of another Mary, a disciple of Christ. CCC 500

Our words

In Hebrew and Aramaic there was no separate word for cousin, so “brother” covered relatives. And at the cross Jesus entrusted His mother to John, which would be odd if she had other sons.
